Common Causes of All-on-4 Failure
The most frequent causes we see are peri-implantitis (infection destroying bone around implants), poor implant positioning due to inadequate planning, insufficient bone reduction before placement, cheap acrylic prostheses that degrade and harbor bacteria, and premature loading before osseointegration is complete. The Revision Process
We begin with comprehensive CBCT 3D imaging to assess remaining bone, current implant positions, and extent of damage. The revision may involve removing failed implants, treating infection, bone reconstruction, placement of new implants (potentially zygomatic implants if bone loss is severe), and fabrication of a new zirconia prosthesis.
