Orthognathic surgery repositions the upper jaw (maxilla), lower jaw (mandible), or both to correct skeletal misalignment that cannot be resolved with orthodontics alone. Dr. Kahwach performs the full range of orthognathic procedures — Le Fort I osteotomy, bilateral sagittal split osteotomy (BSSO), genioplasty, and segmental osteotomies — using 3D CBCT imaging and virtual surgical planning for millimeter-level precision.

Conditions Treated

Class II malocclusion (overbite/retrognathia) — recessed lower jaw causing deep bite and weak chin profile. Class III malocclusion (underbite/prognathism) — protruding lower jaw with reverse bite. Open bite — front teeth do not meet when back teeth are closed. Facial asymmetry — one side of the jaw is longer or wider than the other. Obstructive sleep apnea — jaw advancement to open the airway (see sleep apnea surgery).

The Surgical Process

Treatment is coordinated with your orthodontist — braces align the teeth within each jaw, then surgery repositions the jaws into their correct relationship. All incisions are made inside the mouth, leaving no visible facial scars. Titanium plates and screws hold the jaws in their new position during healing. Hospital stay is typically one night. Most patients return to work or school within 2–3 weeks.

Virtual Surgical Planning

Dr. Kahwach uses CAD/CAM technology to perform the surgery virtually before the operating room. 3D models of your skull are segmented, repositioned digitally, and custom cutting guides are 3D-printed to transfer the plan precisely to surgery.
